Zest Pediatrics is now enrolling patients who have AmeriHealth Caritas Ohio Medicaid insurance at our Solon and Westlake offices. If you don’t have AmeriHealth Caritas insurance, that is ok. You can change to it. It’s simple!

How to Change to AmeriHealth Caritas Ohio
Medicaid Insurance
You may change your Medicaid coverage to AmeriHealth Caritas Ohio by requesting a Just Cause Application. Follow this process:
1. Call Ohio Medicaid Member Services at 800-324-8680.
Ask for a Just Cause Application to change your Medicaid managed care plan. Explain you want to see Dr. Michael Perisa (Solon) or Dr. Meghan Lynch-Ljubi (Westlake) and that the doctor is only in-network for Amerihealth Caritas Ohio. You request to change your Medicaid plan to AmeriHealth Caritas Ohio.
2. The application will take some time to be approved.
3. Once approved, contact Zest through this site to schedule your child’s evaluation.
